What Happens After You Leave Hospital?

Discharge often comes with a long list of instructions covering wound care, medication timing, mobility limits and follow-up appointments, all to manage while still recovering. Our nurses and caregivers step in during this period to carry some of that load, making sure instructions from your surgeon or GP are followed closely.

How Long Does Support Usually Last?

The length of care depends entirely on the procedure and your individual recovery. Some patients need daily visits for a week or two, while others require ongoing support over several months. We review progress regularly and adjust the plan as your needs change, working towards greater independence rather than long-term dependence on care.

How Do You Prepare for Support at Home?

Before discharge, it helps to share your surgeon's instructions and any prescribed medication with our team, so a plan can be ready before you arrive home. A quiet, accessible space to rest, along with any mobility aids recommended by the hospital, also makes the first few days considerably easier to manage.

Our nurses stay in close contact with your surgical team throughout, so any concerns are raised and addressed without delay. Where recovery involves more complex needs, this service often continues into Registered Nursing Care, Wound Care, Catheterisation and Colostomy Support, or Medication and Injection Administration, depending on what your recovery requires.
