Changes in health often show up gradually, in small shifts to blood pressure, temperature or pulse, long before they become obvious. Gwenchana Home Based Care and Occupational Services monitors these vital signs at home, so changes are caught early and shared with your GP without delay.

What Do We Monitor?

Who Needs Regular Vital Sign Checks?

Regular checks can be helpful for people living with conditions such as high blood pressure or diabetes, as well as those recovering from surgery or illness. They may also be useful for older people whose health needs a little closer watching, particularly if they live alone.

What Happens If Something Looks Off?

If a reading is outside the expected range, the nurse will take note and, where needed, contact your GP for further guidance. Keeping a regular record of readings can also make it easier to spot changes over time and gives your GP a clearer picture of what has been happening between visits.

How Often Are Checks Needed?

This depends on your condition and what your GP has advised. Some patients need daily checks for a short period, while others are monitored weekly as part of longer-term management. We work to whichever schedule your GP recommends and adjust it if your health changes.

Monitoring is often arranged alongside Registered Nursing Care or a GP Medical Care visit, so any changes are picked up and acted on quickly by the same team.
